SMS Terms & Conditions
These terms cover the Elite Anesthesia care-coordination text messages program operated by Elite Anesthesia Associates, PLLC (3401 W. Thanksgiving Way, Suite 300, Lehi, UT 84043).
What we send
When you opt in, Elite Anesthesia sends text messages about your care only: appointment reminders, links to your pre-anesthesia medical form, clearance follow-ups, and payment links. We do not send marketing or promotional messages.
How you opt in
You can opt in online at pt.eliteanesthesia.com/sms-consent, or in person or by phone at your referring practice or with Elite staff. Every new recipient receives a confirmation message first. Consent is not a condition of receiving care.
Message frequency and cost
Message frequency varies with your care. Message and data rates may apply according to your mobile carrier's plan. Carriers are not liable for delayed or undelivered messages.
Opting out
Reply STOP to any message to opt out at any time. You will receive one final message confirming that you have been unsubscribed. You can also call 385.832.7232 to opt out.
